PATIENT RIGHTS & RESPONSIBILITIES

PATIENT BILL OF RIGHTS

The EUC Dental Clinic accepts all patients with dental needs which are considered appropriate for teaching students contemporary techniques of dentistry regardless of race, color, creed, religion, national origin, gender, age, marital status, disability, public assistance status, or sexual orientation. However, we reserve the right to refuse treatment, except to relieve pain or protect life, to any person for the following reasons:

  • The patient has a history of not abiding by the patient responsibilities listed below.
  • The patient’s conduct is disruptive or harms the students’ education.
  • The patient has a history of not fulfilling his or her financial obligations to the EUC Dental Clinic.

Your rights as our patient are important to us and we encourage you to let us know if you feel we have not lived up to our pledge or if you feel that your rights were not honored. After your initial appointment, you will have one or more additional appointments to determine your personal treatment plan depending on the complexity of your case.

PATIENT HAS THE RIGHT:

  • Τo be provided with appropriate information about providers, policies and procedures;
  • Τo be informed by your dentist regarding your diagnosis, treatment and prognosis in terms you can understand and you have the right to ask any relevant questions;
  • Τo receive enough information from your dentist to enable you to give informed consent before beginning any dental procedure or treatment;
  • Τo be treated with respect, dignity and with recognition of your privacy;
  • Τo refuse treatment, medication or other procedures recommended by EUC Dental Clinic providers and to the extent permitted by law and to be made aware of the potential consequences of refusing recommended treatment;
  • Τo expect that all communications and records pertaining to your health will be handled in the most confidential manner;
  • Τo express any concern with the staff of the EUC Dental Clinic.

PATIENT HAS THE RESPONSIBILITY:

  • Τo provide, to the best of your knowledge, accurate and complete information about your present health, dental complaints, past illnesses, hospitalizations, medications and other matters pertaining to your health;
  • Τo report any changes in your health since your last appointment to your attending dental student;
  • Τo tell your dental student if you do not understand the treatment plans developed for you or if you do not understand the course of your treatment or what is expected of you;
  • Τo treat others with the same respect and courtesy that you expect for yourself;
  • Τo weigh the potential consequences of not following the advice of your dental care provider;
  • Τo keep and follow the scheduled appointments with providers or give adequate notice of cancellation to the EUC Dental Clinic;
  • Τo express your concerns to the EUC Dental Clinic staff;
  • Τo provide the information needed by your dental care provider to enable him/her to provide the most appropriate and effective care;
  • Τo meet your financial obligations in a timely fashion;
  • Τo perform visual in-mouth examinations on yourself and to responsibly follow the provider’s instructions regarding home care treatment;

Note: Periodically, EUC Dental Clinic will conduct patient surveys as part of our continuous improvement initiatives. By completing these surveys, you contribute to making these services better.
